Our admission process is designed to be straightforward and efficient. It typically takes approximately 20 minutes to complete all necessary steps.What is the difference between personal care facilities, assisted living facilities, intermediate care, and skilled nursing facilities?
Assisted living residences offer adaptable services, allowing residents to progress to nursing home care when needed. Personal care homes transfer residents to skilled nursing facilities if their health needs exceed the available care. Skilled nursing facilities provide temporary, intensive 24-hour medical rehabilitation to prepare residents for more independent care. Assisted living or intermediate care typically helps with daily activities such as bathing, dressing, grooming, medication management, and meal preparation.What medical paperwork will I need?
Prior to admission, Eastern Star Home in Kentucky requires a history and physical (H&P) report. This can be obtained from either the resident's primary care provider or their current living facility.What other paperwork will I need?
In addition to medical documentation, we require copies of the power of attorney document, living will, and insurance cards.What happens when the patient's funds run out?
If a resident's funds are depleting, we encourage open communication. Our team can assist in exploring alternative arrangements while the power of attorney initiates the Medicaid application process.What is a private pay facility?
A private pay facility refers to an arrangement where room and board fees are paid privately. Depending on the type of care required and individual circumstances, skilled nursing costs may be covered by long-term care insurance or personal funds.Do you have a waiting list?
Eastern Star Home in Kentucky is currently accepting resident applications. We have over 40 beds available, including an 18-bed intermediate care facility and a 24-bed personal care home, both licensed by the Commonwealth of Kentucky.How do I transition from the hospital to your care facility?
Transitioning to our facility is a simple process. You can initiate it by contacting us directly or applying online. Alternatively, you may request your hospital discharge planner or case manager to contact us on your behalf to ensure a seamless transition.What to do when you are not covered by Medicaid/Medicare, but you need an elder care facility?
